I would like to do something a little more professional form my photography business and have some nice order forms made. Maybe even the kind that has the perforated envelope at the bottom. I have tried the vista print with no luck, and a few others. Thanks. I have, but not much luck. I have always made my own in photoshop, maybe what I should be looking for is blank pages with the perforated envelope?

Any printing service can do that, but have you checked Mircosoft for the templates they have for making such forms?

I really liked this made copies and slight adjustments to fit it to my needs. It also provided me with a bunch of stuff that I hadn't thought to include in my contracts
